All participants in Grand Valley State Summer camps must fill out a medical information form to bring with the registration. The form can be downloaded and printed here.

Grand Valley State University Tennis 2004
This camp was created to give specialized instruction to beginning, intermediate, advanced and high school tennis players in a fun filled environment. Campers will have daily instruction as well as supervised match play and other tennis games. Our small staff/student ratio of 1:6 will help insure that each camper received individual attention.

Facilities:
The Laker Tennis Center located on the campus of Grand Valley State University will serve as the host facility for the 2004 Laker Tennis Camp. The Tennis Center features 10 outdoor tennis courts as well as a practice hitting wall. In the event of inclement weather indoor courts will be utilized. Grand Valley State University is located 10 minutes west of Grand Rapids in Allendale, Michigan.

Staff:
Camp Director: John Black is the head men's and women's tennis coach at Grand Valley State University. He is a member of both the United States Tennis Association (USTA) and the United States Professional Tennis Association (USPTA).
Coach Black graduated from the College of New Jersey where he was an all-conference member of the nationally ranked men's tennis team. He finished his career at TCNJ with a 42-3 singles record and a 25-4 doubles record. Coach Black also is a tennis professional at Ramblewood Tennis and Health Club located in Grandville, MI.

Camp Staff:
The staff for all camp programs includes Coach Black, fellow college and high school coaches as well as current and former collegiate tennis players.

What to Bring
All campers should bring a tennis racket, comfortable tennis clothes, bathing suit, sunscreen, hat or cap, key or combination lock, and a water bottle. Overnight campers will need to bring all of the above as well as after tennis clothing and personal hygiene items. Cost of the overnight camp includes meals(breakfast, lunch, and dinner) as well as evening entertainment. Day campers will have to provide their own lunch.
